60 West is a specialized skilled nursing facility located in Rocky Hill, Connecticut, established in May 2013. It is a unique center, described as the first of its kind in the country, developed in collaboration with the State of Connecticut to care for individuals who may be transitioning from state care or are otherwise difficult to place in traditional nursing home settings. It serves a diverse population with conditions such as acquired brain injuries, Alzheimer's, and other memory or cognitive disorders. Referrals for admission can come from the community, inpatient psychiatric facilities, and state correctional institutions. The admission process involves a comprehensive assessment and approval by a utilization management committee comprised of staff from the Department of Mental Health and Addiction Services (DMHAS), the Department of Correction (DOC), and 60 West.
