Click here to read more about utilizing professional support during a pandemic.​ How We Help Comprehensive Assessment We begin with a thorough assessment of needs of the elder, including a realistic evaluation of the individual's functioning within their environment, and an understanding of their physical, cognitive, financial and social well-being. Advocacy & Communication We will arrange for any services needed, service recommendations and referrals. In turn, our agency will monitor them to assure the plan is followed, with revisions as needed. Coordination of all needed services occurs within the home, community, and care facilities. Professional counseling to the elder and his or her family regarding appropriate senior living options and home care is also offered during times of transition. Recommendation & Monitoring  We act as a liaison with the family, long distance care givers, and other professionals. Ongoing consultation may continue if necessary to provide a reliable link to long distance caregivers. Our agency also provides advocacy for elders in nursing home placements in addition to active monitoring of care and medical records while communicating with providers.
