Welcome to Paonia Care and Rehab Center Paonia Care and Rehabilitation Center is a licensed 60-bed skilled nursing and rehabilitation facility located on the western slope of Colorado in the beautiful town of Paonia. Our center provides short term rehabilitation, long-term care, outpatient rehab services, and a secure memory unit. Nestled in the North Fork Valley, a rural area surrounded by the magnificent West Elk mountains, we are more than a mile-high city as our altitude is close to 6,500 feet above sea level. We are a community that is diversified with coal miners, farmers, cowboys, and retirees. Our residents and families enjoy spectacular courtyard views and fun events with the locals. Available Positions Services Rehabilitation Services We are your short-term & post-hospital rehabilitation provider. Physical Therapy Physical therapists will develop a plan to begin restoring your range of motion and mobility. Occupational Therapy We use task-directed activities in the treatment of our residents whose functional abilities have been impaired. Speech Therapy Our goal is to help restore abilities in the areas of communication, swallowing and cognition. Rehabilitation We are your short-term & post-hospital rehabilitation provider.
