These a la carte services can include one-time consultations to address specific challenges, family education and coaching on caregiving and medical decision-making, and guidance during post-hospital recovery or following a new diagnosis. They also provide welfare visits and medical advocacy for families living out of the area. A key aspect of their service is assisting with housing transitions when staying at home is no longer a safe option.
