Advanced Illness Management AIM is a palliative Care Program administered through Home Health. Hospice Care Compassionate care provides comfort and support and works to preserve dignity and improve the quality of life for those dealing with a life-limiting illness. Mental Health Home Care Our integrated care program is personalized to help each patient flourish in the home setting and beyond. ELIGIBILITY, INSURANCE, AND PAYMENT Concerned Care Home Health and Hospice is committed to providing compassionate care. As part of the social services we offer financial and home assessment. We also assist you with community resource referral options. Our staff will assist you in determining your insurance eligibility and coverage before we begin services.
