Contact Us Donations Services Photo Gallery Employee Resources Menus Activities AN ALTERNATIVE TO NURSING HOME CARE All Care is a Case Management Organization dedicated to serving the aged and disabled of West Virginia. We have several offices located in the state. Case Management All Care's person centered case management includes the coordination of services that are individually planned and arranged for members and helps to avoid duplication and provision of unnecessary services, and to ensure a balance of services. The Case Manager serves as an advocate and coordinator of care for the member. This involves collaboration with individual, family members, friends, informal supports, health care and social service providers. Social, environmental , service, and support needs are evaluated. A person centered service plan is developed and written as needed and details all services that are to be provided. The Service Care Plan includes services to assure the member remains safely in their home. The Case Manager proactively identifies problems and coordinates services that provide appropriate high-quality care to meet the individualized and often complex needs of the member. MEDICAID WAIVER The Aged and Disabled Waiver Program provides in-home and community services to individuals 18 years of age and older who are medically and financially eligible. Services include Case Management, Homemaker, Transportation, and annual Registered Nurse assessment and review. TAKE ME HOME WEST VIRGINIA ****This process must be started before release from nursing home.****** Take Me Home provides transition services and support to eligible individuals who are elderly, physically disabled or have serious mental health issues from facility based living to their own homes and communities.
