PCA services are aimed to help support persons with disabilities in continuing to live independently. Contact our team to learn more about your options! Basic Support and In-Home Support Services Diamond Lake Home Healthcare is a licensed Home and Community Based Services (HCBS) provider. We provide supervision, care, and assistance in order for our clients to continue living independently in their own homes. Our services include: Adult Companion Services: Non-medical supervision and socialization 24hr Emergency Assistance: On-call counselling or immediate response for assistance due to health/personal emergency Homemaker Services: Household cleaning, home management, and/or assistance with daily living tasks Individual Community Living Support (ICLS): For people who need moderate supervision/assistance to remain in their own homes Night Supervision: Overnight assistance and monitoring in person's home Respite Care: Short-term relief for primary caregivers Individualized Home Supports w/o Training: Support, assistance, and/or supervision in community participation, health/safety/wellness, household management, and adaptive skills. w/ Training: Our caregivers will provide support as well as training or habilitation in the above-mentioned categories. w/ Family Training: Our caregivers will provide support and training to person and their families Frequently Asked Questions What types of services do you offer?
