We provide the following services for those patients meeting the Medicare guidelines and eligibility requirements. Skilled Nursing Physical Therapy Speech Therapy Occupational Therapy Home Health Aid Care Medical Social Workers Medical Equipment and Supplies Coordination Medicare and Patient Eligibility Guidelines: 1. Care must be Intermittent 2. Must be homebound – (Confined to the home due to illness or injury. Infrequent absences from home are allowed primarily for medical care. Homebound patients need not to be bedridden, but leaving their residence must require considerable effort to supportive devices such as cane, wheelchair, or the help of another person 3. Must be under the care of a physician who provides the home health agency with written orders for home care 4. Must require skilled care 5. Receive care in their place of residence, This may be in their home, the home of a friend or relative or other residence but not a hospital or nursing home. Services may also be limited if the patient lives in a boarding home or adult residential facility.
