We offer team-based primary care wherever you call home, hospice services for both our primary care patients and other adults in the Portland metro area, and community-based palliative care support for seriously ill members enrolled in Medicaid through our advanced illness care program. Together, all three of our programs are proving that in home medical care for seriously ill patients can improve health outcomes, increase patient comfort, reduce health care costs and contribute to care team satisfaction.
