Indeed, for some, it can mean the difference between being able to remain in their own home and having to relocate to an assisted living facility. Impact NW values the independence of seniors and adults with disabilities. In partnership with Multnomah County ADS, TriMet, and Ride Connection, Impact NW’s Transportation Assistance program makes it easy for our clients to get to medical appointments, do their shopping, and participate in social activities that keep them engaged in their community. Annually, our program provides more than 14,000 rides, and the coordination of thousands more. Can we help you? Our Transportation Services: Escorted Rides: Escorted door-to-door transportation services are offered to SE Portland seniors and adults with disabilities who need assistance getting to medical, dental and personal appointments. Bus tickets and/or taxi rides: Monthly TriMet bus/lift tickets are available for Impact NW senior and case management clients, based on qualification and availability. Group nutrition rides: Impact NW drivers provide group rides to major grocery stores fives times a week, as well as to select food banks. Food deliveries are provided to home-bound seniors. Recreational transportation: Impact NW drivers take groups of seniors and adults with disabilities on recreational trips planned by one of our activities coordinators.
