Aging, Disability, and Transit Services of Rockingham County Aging, Disability, & Transit Services (ADTS) has served Rockingham County older adults, individuals with disabilities, and their caregivers for over 50 years. Our mission is to enhance the quality of life for individuals by empowering them to achieve optimum health and well-being, independence, and participation in the community. We accomplish our mission by offering community-based services that support their choice to remain in their home and community until they choose otherwise. Community-based services include: Our Services Home Care Our dedicated team of Registered Nurses will work with your family to develop a personal plan of care that addresses your individual and unique needs. Senior Nutrition Senior Nutrition Services provides older adults a nutritious meal, social engagement, and a wellness safety check. We offer more than just a meal; it is a celebration of togetherness. (LEAF) Adult Day Program Our goal at LEAF is to provide opportunities for life enrichment and friendship through engaging activities that focus on wellness and individual strengths rather than limitations. Community Alternative Program The Community Alternative Program for Disabled Adults (CAP/DA) is a Medicaid program designed to give disabled adults (age 18 and over) who are at risk of unwanted nursing facility placement, the option of remaining safely at home with a package of services to complement their current family support.
