AOS Care Management
Founded in 1999 by gerontologist Amy Natt, Aging Outreach Services (AOS) is a full-service elder care firm based in Southern Pines, North Carolina. What began as a small care management practice has grown into an organization that provides comprehensive support to older adults and their families throughout Moore County and eight adjacent counties, including Chatham, Cumberland, Harnett, Hoke, Lee, Montgomery, Richmond, and Scotland. AOS operates with a mission to provide direct care and community support to older adults facing the challenges of aging, with a particular focus on those with dementia. The organization is built on a foundation of advocacy and action, aiming to help individuals age with dignity and choice, and remain safely in their homes whenever possible. AOS Care Management is a key component of the organization, consisting of a team of professionally trained Aging Life Care™ Managers. These professionals take a holistic, client-centered approach to geriatric care management, offering services such as assessments and evaluations, care planning, placement assistance, and crisis intervention. A unique aspect of AOS is its AOS At Home Care service, which is the first accredited caregiver registry in North Carolina. This service connects clients with a network of private duty caregivers who have been carefully screened, interviewed, and have had their credentials verified. This consumer-directed model allows clients to choose their caregivers and determine the scope of services and schedules that best fit their needs. AOS also offers the 'AOS Cares' program, which provides clients with 24/7 access to a care manager for emergencies and after-hours support. Beyond direct care services, Aging Outreach Services is deeply invested in the community. Team members are active on local boards, lead support groups, and provide educational programming through a speakers guild. The organization also publishes a monthly educational resource called OutreachNC and has a YouTube podcast called 'All Things Aging'. In 2015, the non-profit wing of the organization, AOS & Friends Care, Inc., was established to further support older and disabled adults and their caregivers, with a special focus on those living with dementia and memory impairment. This non-profit was co-founded by Amy Natt and John Barrett, a family advocate.
