Home Health Home health services are designed to support a safe and smooth transition back to your home following discharge from your most recent inpatient stay. To ensure continuity of care, our team completes an Initial Assessment within 48 hours of discharge. Your Primary Care Physician may also refer you for home health services if you are homebound and have a medical necessity. Through scheduled, intermittent visits, our skilled clinical staff provides the care and oversight needed to promote recovery—allowing you to heal safely and comfortably at home.
