At Grayce we provide care that focuses on quality of life, the patient's and family's wishes, and easing distress at the end of life and the months preceding death. Grayce can explain what hospice is and how we can help. We believe in hospice care that enhances and increases a patient’s quality of life while providing comfort and support for their family. Our Services • Regular nursing visits to maintain the patient’s comfort, including regular assessments and support for the family. • Enhanced care plan for pain and symptom management. • Assist patients with personal care and activities of daily living. • Provide needed medications, medical supplies, and medical equipment. • Instruct families and caregivers on how to properly care for the patients. • Social workers provide emotional support, supportive counseling, information and referral services, assistance with advanced directives, out-of-home placements, etc. • Chaplains provide support and help patients and families with spiritual questions or concerns at the end of life. • Volunteers to provide companionship, emotional support, and activities. • Bereavement services and counseling to help families deal with grief. • Access to Hospice Nurses 24 hours a day, 7 days a week.
